Why Women Experience Different Heart Attack Symptoms

7 August 2026 by My Durban

Affinity Health, a leading provider of high-quality health cover, is encouraging women to learn the often subtle warning signs of a heart attack this National Women’s Day (9 August), as symptoms can differ significantly from those commonly experienced by men.

Heart attack symptoms are not always what people expect. Most people still believe that a heart attack always involves sudden, severe chest pain. While chest discomfort remains one of the most common warning signs, women are more likely to experience symptoms that are less obvious and easier to overlook.

These symptoms can include unusual fatigue, shortness of breath, nausea, dizziness, jaw, neck, back, or shoulder pain, and even flu-like symptoms. Because they do not fit the traditional image of a heart attack, these symptoms are often mistaken for stress, indigestion, anxiety, hormonal changes, or a minor illness. As a result, there may be a delay in seeking medical attention, increasing the risk of serious complications.

Are Heart Attacks Different in Women?

A heart attack occurs when blood flow to part of the heart is blocked, preventing oxygen-rich blood from reaching the heart muscle. Without prompt treatment, permanent damage can occur.

Although both men and women can experience chest discomfort during a heart attack, women are more likely to experience additional symptoms that do not immediately seem related to the heart.

Common Heart Attack Symptoms in Women

Women may experience:

  • Chest pressure, tightness, squeezing, or discomfort.
  • Shortness of breath.
  • Pain in the neck, jaw, shoulders, upper back, or arms.
  • Nausea or vomiting.
  • Dizziness or light-headedness.
  • Cold sweats.
  • Unusual or overwhelming fatigue.
  • Pain or discomfort in the upper abdomen.
  • A feeling that something is simply “not right”.

While some women experience severe chest pain, others report only mild discomfort or pressure. In some cases, symptoms may develop gradually over several hours, days, or even weeks.

The Warning Sign Many Women Miss

One of the most commonly overlooked heart attack symptoms in women is unusual fatigue.

This is not the normal tiredness that comes after a busy day. Instead, it can feel overwhelming and persistent. Some women report feeling exhausted after completing simple tasks such as climbing stairs, carrying groceries, or doing household chores.

Others experience fatigue for days or weeks before a heart attack occurs.

Because tiredness is often blamed on stress, poor sleep, work pressures, or family responsibilities, many women do not realise it could signal a serious heart problem.

Why Do Women Experience Different Symptoms?

There are several reasons why heart attack symptoms may differ between men and women.

Women are more likely to develop disease in the smaller arteries that supply blood to the heart, a condition known as coronary microvascular disease. These smaller blood vessel problems can produce different symptoms compared to blockages in the larger coronary arteries more commonly seen in men.

Hormones may also play a role. Oestrogen helps protect the heart during the reproductive years, but this protection decreases after menopause. As women get older, their risk of heart disease rises significantly.

In addition, conditions that affect women more frequently, such as autoimmune diseases, pregnancy-related complications, and hormonal changes, may influence heart health and how symptoms present.

Risk Factors Women Should Know About

Several factors increase a woman’s risk of heart disease and heart attacks, including:

  • High blood pressure.
  • High cholesterol.
  • Diabetes.
  • Smoking.
  • Obesity.
  • Physical inactivity.
  • Chronic stress.
  • Family history of heart disease.
  • Pregnancy complications such as pre-eclampsia.
  • Early menopause.

Diabetes deserves special attention, as women with diabetes face a particularly high risk of heart disease compared to women without the condition.

Don’t Ignore the Signs

One of the biggest challenges in women’s heart health is that symptoms are often dismissed or explained away.

Many women assume they are too young, too healthy, or too busy to have a heart attack. Others may attribute symptoms to anxiety, menopause, digestive problems, or simply getting older.

However, when it comes to a possible heart attack, it is always better to seek medical attention and be wrong than to stay home and risk serious complications.

If you experience chest discomfort, unexplained shortness of breath, sudden sweating, dizziness, nausea, unusual fatigue, or pain spreading to the jaw, neck, back, shoulders, or arms, seek urgent medical help immediately.

Protecting Your Heart

The good news is that many heart disease risk factors can be managed.

Women can help protect their heart health by:

  • Having regular health screenings.
  • Monitoring blood pressure and cholesterol levels.
  • Managing diabetes effectively.
  • Exercising regularly.
  • Maintaining a healthy weight.
  • Eating a balanced diet.
  • Avoiding smoking.
  • Managing stress levels.
  • Knowing their family medical history.

Heart disease remains one of the leading causes of death among women worldwide, but greater awareness can help save lives.

Conclusion

Heart attacks do not always look the same, especially in women. Understanding the warning signs, recognising risk factors, and seeking medical attention without delay can make a life-saving difference. This National Women’s Day, let this serve as a reminder that heart health should never take a back seat.
